Physician Directors – Sean Devlin DO, MS, MD(H) and Jason Chen DO, IFMCP

Role and Responsibilities:

  • Oversees and coordinates your entire care plan.
  • Diagnoses complex medical conditions and reviews your laboratory and diagnostic results.
  • Develops advanced treatment strategies using both conventional and functional medicine approaches.
  • Guides the care team to ensure all therapies align with your goals and medical needs.

Capabilities:

  • Board-certified in conventional medicine with advanced training in functional and integrative health.
  • Expertise in complex case management, root-cause analysis, and regenerative therapies.
  • Ability to prescribe medications, order and interpret advanced diagnostic tests, and perform specialized procedures when necessary.

Nurse Practitioner – Natalie Duperreault-Rohde

Like our Physicians, our Nurse practitioners are licensed to diagnose, prescribe and administer therapeutic interventions. They will typically manage your care plan with consistent consultation with physician directors to ensure we optimize your personalized health pathway.

Role and Responsibilities:

  • Conducts detailed evaluations, health assessments, and regular follow-up visits.
  • Implements and manages your personalized treatment plan, including prescription management, lifestyle interventions, and nutrient therapies.
  • Collaborates closely with the physician director to adjust care plans based on your progress.
  • Provides education on disease prevention, lifestyle medicine, and functional health optimization.

Capabilities:

  • Licensed to diagnose and treat medical conditions independently.
  • Skilled in creating functional health roadmaps focused on nutrition, hormones, gut health, and chronic disease prevention.
  • Can order labs, prescribe therapies, and monitor your overall progress.

Functional Medicine Wellness Coaches – Matt Kinneberg and Madison Miller

Letโ€™s face facts, we all sometimes need a little nudge in life. Additionally, deciphering the vast amount of information we receive across all facets of our lives can be overwhelming. Organizing and prioritizing can feel impossible even for the most successful. Administrative assistants, personal assistants, accountants, babysitters, and cleaning people help us stay on track and maintain order in their niches of our day. Why wouldnโ€™t you have an expert on your health to do the same? In comes your wellness coach!

Role and Responsibilities:

  • Coaches you through nutrition, exercise, stress management, sleep optimization, and habit-building strategies.
  • Helps you set achievable goals and provides ongoing accountability and encouragement.
  • Acts as a liaison between you and the clinical team to keep your care plan practical and personalized.

Capabilities:

  • Certified in behavior change, motivational interviewing, and functional nutrition principles.
  • Skilled in creating and supporting client functional health roadmaps focused on nutrition, hormones, gut health, and chronic disease prevention.
  • Trained to work within the functional medicine model alongside licensed practitioners.
  • Specializes in empowering you to take charge of your wellness pathway and overcome obstacles.

How Your Care Team Works Together

  • Integrated Planning: Your physician, nurse practitioner, and wellness coach meet regularly to review your progress and refine your care plan.
  • Seamless Communication: Updates from every visit โ€” whether with the NP or coach โ€” are shared with the full team to ensure coordinated care.
